Every Monday, we are proud to offer our services Free of Charge or for a Donation of any amount. This initiative is designed to provide those in need with access to our healthcare services, empowering our community towards better health and wellness without financial barriers.

How It Works
- Appointment Booking: Due to the expected high demand, appointments for “Healthcare Access Mondays” are required and will be available on a first-come, first-served basis.
- Service Day: Every Monday, our diagnostic imaging services are free of charge or donations can be made, the amount of which is completely at their discretion.
- Making a Donation: Donations are purely voluntary. Every contribution, big or small, helps us sustain and expand this initiative to reach more members of our community in need.

To ensure our services reach those who need them the most, we ask those interested to complete a basic eligibility application process. More details about this process will be provided upon inquiry.

How to Schedule Your Ultrasound Diagnostic Imaging Appointment
To ensure a seamless process for scheduling your ultrasound diagnostic imaging appointment, please follow the steps outlined below:
Step 1: Obtain a Physician Order
Your first step is to acquire an order for your ultrasound from your physician. This document is crucial as it outlines the need for diagnostic imaging and specifies the type of ultrasound required.
Step 2: Upload the Physician Order
Once you have obtained the physician order, please proceed to upload the document using the upload field provided below. Make sure the document is clear and all the information is readable.

Step 3: Start Your Appointment Process
After successfully uploading the physician order, you can begin the appointment process by clicking on the “Start Your Appointment” button. This action will redirect you to the Patient Registration Form accessible through DocuSign.
Step 4: Complete the Patient Registration Form
Please take your time to accurately fill out the Patient Registration Form. It’s crucial that all the information provided is complete and accurate to avoid any delays in processing your request.
Step 5: Await Confirmation
Upon submission of your Patient Registration Form, one of our dedicated staff members will review your submission and contact you to finalize the details of your appointment. They will assist you in arranging a convenient time for your ultrasound diagnostic imaging.
